REQUIREMENTS FOR DRUG TESTING:

Consent from the Testee
Referral Letter from Work Place (Optional)
Referral Letter from Academic Institution (Optional)
Letter of Consent from Parents if a Minor
Letter from Embassy in case of International Travel (Optional)
Doctors Prescription if on Medication
Urine Sample of the Testee at the Point of Testing
Remita Retrieval Reference and Receipt of Payment N10,000/N5,000 (as the case may be) in the name of National Drug Law Enforcement Agency (NDLEA).
